ABSTRACT

Abstract Introduction: Obesity has been shown to be associated with low levels of soluble receptor for advanced glycation end products (sRAGE). Objective: To evaluate the levels of sRAGE and its association with the lipid index in children with obesity. Methods: Cross-sectional study of children with obesity aged between six and 11 years. Anthropometric measurements, glucose, lipid profile, insulin and sRAGE were evaluated; body mass index, total cholesterol/high-density cholesterol (TC/HDL-C), triglycerides/glucose (TG/glucose), and triglycerides/HDL-C (TG-HDL-C) ratios and HOMA-IR were also calculated. Results: Eighty children were studied, among which 50% were males and 50% females. Females had higher values for waist circumference, HOMA-IR, and TG/HDL-C and TG/glucose ratios. No significant differences were found for sRAGE. When the variables were compared according to TG/HDL-C ratio tertiles, higher TC/HDL, TG/glucose, and sRAGE values were found at upper tertile. A significant correlation was observed between sRAGE and HOMA-IR (p < 0.03) in males, and between sRAGE and TG/HDL-C (p < 0.01) and TG/glucose ratios (p < 0.008) in females. Conclusions: The female gender showed more cardiovascular risk factors and higher sRAGE at TG/HDL-C upper tertile. Further studies are required to test the possible predictive effect of higher risk for developing metabolic and cardiovascular complications.

ABSTRACT

ObjectiveTo explore the clinical effect of Tiaoxin formula in the treatment of patients with coronary heart disease and anxiety/depression and its impact on serum levels of 5-hydroxytryptamine (5-HT), β- thromboglobulin (β-TG) and myeloperoxidase (MPO). MethodA total of 66 patients with coronary heart disease and anxiety/depression were randomly divided into the Tiaoxin formula group and Deanxit group, 33 cases in each group. Both groups were given fundamental western treatment for coronary heart disease. Additionally, the Deanxit group was treated with flupentixol and melitracen tablets and the Tiaoxin formula group was treated with Tiaoxin Formula. The treatment lasted 8 weeks. Before and after treatment, the changes of clinical efficacy, Patient Health Questionnaire (PHQ-9), Generalized Anxiety Disorder (GAD-7) scale, Seattle Angina Questionnaire (SAQ), heart rate variability, and serum 5-HT, β-TG and MPO levels, and incidence of adverse reactions in the two groups were observed. ResultThere was no significant difference in the baseline indexes of patients in the two groups, and thus the two groups were comparable. After treatment for 8 weeks, the total effective rate for traditional Chinese medicine (TCM) syndromes in the Tiaoxin Formula group was 87.88% (29/33) higher than 63.64% (21/33) in the Deanxit group (Z=-2.653, P<0.05). Compared with those before treatment, the PHQ-9 and GAD-7 scores of the two groups were decreased at week 4 and 8 of treatment (P<0.05), and there was no statistical difference between two groups. And the SAQ dimension scores of the two groups were increased at week 4 and 8 of treatment (P<0.05). Compared with the Deanxit group, the Tiaoxin Formula group had elevation in two dimension scores: Physical limitation and angina stability (P<0.05). Compared with the conditions before treatment, the serum 5-HT level in the two groups were increased, while the β-TG and MPO levels were lowered (P<0.05), and there was no distinct difference between two groups. In addition, the standard deviation of normal-to-normal intervals (SDNN) and standard deviation of average normal-to-normal intervals (SDANN) of the heart rate variability in the Tiaoxin formula group were elevated after treatment (P<0.05), which were more significant than those of the Deanxit group (P<0.05). During the treatment period, the incidence of adverse drug reactions in the Tiaoxin formula group was lower than that in the Deanxit group (P<0.05), and no adverse events were observed in the two groups. ConclusionTiaoxin formula was effective for the treatment of patients with coronary heart disease accompanied by anxiety and depression, which improved the clinical symptoms, increased serum 5-HT levels, and decreased serum β-TG and MPO levels, and had few adverse reactions and high safety for patients, showing a high clinical value.

ABSTRACT

OBJECTIVE@#In this study, the results of forward and reverse blood typing of a male patient diagnosed as bronchiectasis were inconsistent, which were type O and type A respectively. Multiple experiments including genotyping and sequencing and family investigation were carried out to determine the subtype of ABO blood group and explore the serological characteristics of this subtype.@*METHODS@#Standard serological techniques were used to conduct forward and reverse typing, reverse blood typing enhancement test, H antigen identification, absorption-elution test, salivary blood group substances test, and PCR-SSP method for ABO genotyping and exon 6 and 7 sequencing.@*RESULTS@#The proband's blood group was type O by forward typing, but antigen A could be detected by absorption-elution test, anti-A1 could be detected by reverse blood typing enhancement test, it was found that there was substance H but no substance A in saliva, and the serological characteristics were consistent with Ael subtype. Gene sequencing analysis showed that there was a c.625T>G base substitution on the basis of A102, which had never been reported before. Family survey showed that c.625T>G base substitution appeared in three generations of the family.@*CONCLUSION@#In this study, a new subtype A with Ael serological characteristics caused by c.625T>G mutation was identified. c.625T>G base substitution results in the weakening of A antigen, and this mutation can be stably passed down to future generations.

ABSTRACT

Objective: Management of low-density lipoprotein cholesterol (LDL-C) and triglyceride (TG) is important for patients with type 2 diabetes merger hyperlipidemia. Pemafibrate (PF) has different characteristics from conventional fibrates. In this study, we retrospectively compared the efficacy and safety of PF and bezafibrate (BF) in patients with type 2 diabetes merger hypertriglyceridemia. Methods: Patients who were administered PF (0.2 mg/day) or BF (400 mg/day) for 24 weeks or longer were included. Twenty patients in each group were extracted using propensity score matching (PS). PS was calculated using the patient background (before the start of administration) of PF or BF. We investigated lipid-related parameters (TG, high density lipoprotein cholesterol [HDL-C], and LDL-C) and other laboratory test parameters pre administration and 24 weeks post administration. Results: TG decreased significantly in both groups (p<0.05). However, there were no significant differences between the two groups in the TG treatment target (<150 mg/dL) achievement rate (p =1.00), TG change rate (p=0.84), and TG change amount (p=0.77). In addition, there were no significant changes in HDL-C and LDL-C in both groups. In the PF group, alanine transaminase (ALT) (p< 0.05), alkaline phosphatase (p<0.05) decreased. In the BF group, ALT (p<0.05) and γ-GTP (p<0.05) decreased. Both groups showed improvement in liver function after 24 weeks. eGFR (p<0.05) significantly decreased only BF group. There were no significant changes in renal function, creatine kinase (CK), or hemoglobin A1c (HbA1c) in either group. Conclusion: Our study suggests that there is no difference in the TG lowering effect and safety of PF and BF in type 2 diabetic patients.

ABSTRACT

Objective:Based on the AAPM-TG218 report, the dose verification of intensity-modulated radiotherapy (IMRT) plans were classified to understand the current status, establish the process and determine the limits of dose verification in our hospital. Methods:Different combinations of tumor locations, accelerators, treatment planning systems and verification devices in our hospital were verified and compared to determine the tolerance limits and action limits of each combination. The measurement requirement was adopted according to the AAPM-TG218 report, and 80 cases were selected for each measurement. The measurement procedures were implemented based upon the AAPM-TG218 report and clinical experience of our hospital. Results:The clinical action limits of IMRT plans in our hospital could meet the recommended range of the AAPM-TG218 report, and the tolerance limits were slightly lower than the AAPM-TG218 report′s recommendation (93.94% for 3%/2 mm). The measurement of verification devices was related to the sensitivity. The tolerance limits measured by EPID were higher than ArcCHECK, especially when the dose/distance requirements were more stringent (94.12% and 92.03% for 3%/2 mm, P=0.074; 86.82% and 74.61% for 2%/2 mm, P=0.017). Conclusion:Through the AAPM-TG218 report, the work flow of IMRT dose verification and the limit range are established in our hospital, providing guidance for subsequent clinical dosimetric measurement.

ABSTRACT

ABSTRACT Objectives Graves' disease (GD) is an autoimmune disease causing the overproduction of the thyroid hormone from thyroid gland. This disease is mainly the result of the production of antibodies against TSH receptors. Cytokines play an important role in orchestrating the pathophysiology in autoimmune thyroid disease. The regulatory role of IL-12 on TH1 cells has been proven. IL-27 and IL-35, members of IL-12 cytokine family, are two cytokines that have been newly discovered. IL-35 has been identified as a novel immunosuppressive and anti-inflammatory cytokine while IL-27 has both inflammatory and anti-inflammatory functions. The objective of the current study was to examine the changes in the serum level of the foregoing cytokines in GD patients in comparison to healthy controls. Materials and methods In this study, serum levels of IL-27 and IL-35 were determined by an ELISA method; anti TPO and anti Tg were measured by an RIA method in 40 new cases of Graves's disease. The findings were compared with 40 healthy controls. Results The results showed a significant difference between IL-27 and IL-35 regarding their serum levels with P values of 0.0001 and 0.024, respectively; anti TPO and anti Tg levels of the cases were also significantly different from controls (p < 0.001). Conclusion The reduction in the serum levels of IL-27 and IL-35 in GD patients compared to normal subjects suggests the possible anti-inflammatory role of these cytokines in GD.

ABSTRACT

In Argentina, the prevalence of obesity has increased considerably and type 2 diabetes mellitus (DMT2) reaches 12.7%. Obesity presents heterogeneity and the increase in abdominal fat may include hypertriglyceridemia, hyperglycemia, decrease in HDL-C, increases in apolipoprotein B (Apo B), increase in small and dense LDL lipoproteins, hyperinsulinemia, insulin resistance (IR), chronic inflammatory state, prothrombotic state and other metabolic alterations, which have been included in the so-called metabolic syndrome with 20 to 40% prevalence. Insulin resistance is present in 10 to 25% of the population and is associated with these alterations. The determination of IR is complex; it needs hospitalization and must be performed by specialists. In the laboratory, it can be estimated through insulin dosing, but it is not standardized, so it is necessary to have simple and accessible techniques. The triglycerides (TG)/HDL cholesterol (HDL-C) ratio can be an option using TG/C-HDL cutoff values ≥2.5 in women and ≥3.5 in men. It is significantly associated with IR and CVD and has good specificity but low discriminating power. So when it is present and there is a risk of T2DM or cardiovascular disease, CVD should be complemented with the calculation of non-HDL cholesterol or Apo B and the remaining cholesterol. Considering the pandemic of obesity and DMT2 and the high frequency of CVD, the TG/C-HDL ratio marker should be reported by the biochemical-clinical laboratory.

ABSTRACT

Diabetic retinopathy (DR) is a major microvascular complication of diabetes. It is the most common cause of blindness in the working-age population in developed countries. We wanted to analyse the correlation between risk factors of blindness like duration of diabetes, dyslipidaemia, hypertension, HbA1c with severity of diabetic retinopathy in order to design appropriate strategies for prevention and treatment of diabetic retinopathy.METHODSThis was a retrospective study of all diabetic patients with diabetic retinopathy who presented to the eye OPD at KS Hegde Medical Academy from April 1st 2018 to March 31st 2019 that fulfilled the inclusion criteria. A dilated fundus examination was done to note the grade of diabetic retinopathy. The demographic data along with the duration of diabetes, HbA1c values, Cholesterol levels and Blood pressure were documented and correlated with the severity of diabetic retinopathy.RESULTSThe study included 92 patients, of which 63 were males and 29 were females with a mean age of 54.5±2.8 years. We found that there was statistically significant association between the duration of diabetes and HbA1c levels with severity of diabetic retinopathy (p= 0.022 and 0.034 association), whereas there was no statistically significant correlation between blood pressure and cholesterol levels with severity of diabetic retinopathy (p= 0.52 and 0.456 respectively)CONCLUSIONSDiabetic retinopathy showed a male preponderance, with risk factors like duration of diabetes and HbA1c levels having a significant association with the severity of diabetic retinopathy. Therefore, it is essential to have a good systemic control of diabetes with diet and suitable medications. Diabetic retinopathy is a preventable cause of blindness when diagnosed early and screening of diabetic retinopathy must be done in all diabetics to prevent the progression of the disease.

ABSTRACT

Patient-specific volumetric modulated arc therapy (VMAT) quality assurance (QA) process is an important component of the implementation process of clinical radiotherapy. The tolerance limit and action limit of discrepancies between the calculated dose and the delivered radiation dose are the key parts of the VMAT QA processes as recognized by the AAPM TG-218 report, however, there is no unified standard for these two values among radiotherapy centers. In this study, based on the operational recommendations given in the AAPM TG-218 report, treatment site-specific tolerance limits and action limits of gamma pass rate in VMAT QA processes when using ArcCHECK for dose verification were established by statistical process control (SPC) methodology. The tolerance limit and action limit were calculated based on the first 25 in-control VMAT QA for each site. The individual control charts were drawn to continuously monitor the VMAT QA process with 287 VMAT plans and analyze the causes of VMAT QA out of control. The tolerance limits for brain, head and neck, abdomen and pelvic VMAT QA processes were 94.56%, 94.68%, 94.34%, and 92.97%, respectively, and the action limits were 93.82%, 92.54%, 93.23%, and 90.29%, respectively. Except for pelvic, the tolerance limits for the brain, head and neck, and abdomen were close to the universal tolerance limit of TG-218 (95%), and the action limits for all sites were higher than the universal action limit of TG-218 (90%). The out-of-control VMAT QAs were detected by the individual control chart, including one case of head and neck, two of the abdomen and two of the pelvic site. Four of them were affected by the setup error, and one was affected by the calibration of ArcCHECK. The results show that the SPC methodology can effectively monitor the IMRT/VMAT QA processes. Setting treatment site-specific tolerance limits is helpful to investigate the cause of out-of-control VMAT QA.

ABSTRACT

Objective: To determine the utility of the triglycerides/HDL-C index for the diagnosis of metabolic syndrome (MS) in obese 2-14 years of age children. Methods: Cross-sectional diagnostic study. We included 360 exogenous obese children (199M/161F) divided in three age groups: 2 to <6; 6 to <10 and 10-14 years. MS was defined according to the International Diabetes Federation and the TG/HDL-C index was evaluated using two cutoffs by age group, ≥2.32 and ≥3.5. Chi squared was used accepting a p value <0.05 as statistically significant. Fir each cut-off we determined sensitivity, specificity and predictive values. Results: Frequency of MS was 15.79% in the 2 to <6 years age group; 20.25% in the 6 to<10 years age group and 19.63% in the 10-14 years age group. Among children with SM, 97.1% had low HDL and 83.8% had elevated triglycerides. A statistically significant difference was found in the frequency of the TG/HDL-C index in children with or without MS in all age groups. Sensitivity for all cut-offs was high (86-100%) and specificity was best for the cut-off of ≥3.5 (72-80%). Conclusions: The TG/HDL-C index is a sensitive and specific marker of MS from the first years of age.

ABSTRACT

Dyslipidemia and raised serum hsCRP level occurring in diabetic patients, play an important role in development of macrovascular and microvascular complications as compared to nondiabetics. Keeping in view the prevalence and increased risk of multiorgan involvement in diabetes, it is imperative to diagnose prediabetic individuals and assess their serum lipid profile and serum hsCRP level and Use of these biomarkers as a preventive measure from the complications of prediabetes, developing overt diabetes and increased morbidity and mortality. Aim: observation of serum lipid profile and hsCRP in prediabetic population. Methods: Prospective cross-sectional case control observational studycarried out between august 2017- November 2018 (16 months) which included 100 cases (prediabetics) and 100 controls (nondiabetics). Lipid profile and serum hsCRP level of all prediabetic individuals and nondiabetics was done and statistically analyzed. Results: Serum Total cholesterol, triglyceride (TG), low density lipoprotein (LDL) and serum hsCRP level were significantly raised in prediabetic individuals as compared to nondiabetics, whereas high density lipoprotein (HDL) was nonsignificantly lower and mean with SD was much lower side in prediabetic individuals as compared to nondiabetics. Conclusion: Prediabetics had altered lipid profile as compared to nondiabetics. These prediabetic individuals, because of their raised hsCRP level and dyslipidemia, are at higher risk for developing cardiovascular disease and other multiorgan involvement. So to halt progression by primary prevention as Lifestyle modification or pharmacotherapy in such individuals becomes animportant public health consideration.

ABSTRACT

Technical grade dinitrotoluene (tg-DNT) [CH3C6H3 (NO2)2] nitroaromatic agents which are manufactured in theindustries and applied in both commercial and military in all over the world and Egypt. DNT causes malfunctions inkidneys, heart, liver, testes and mammary glands in animals and human beings, which may be considered as carcinogenic inexperimental animals and human. Moreover, taurine, a free β-amino acid with remarkable antioxidant activity, hasimportant beneficial effects on the human body; hepatoprotection, nephroprotection, cardiovascular protection, hypoglycemic impact and hypolipidemicaction. Currently, taurine level in the serum is used as early marker of breast, endometrial and colon cancers. The current research was aimed to explore the potential impacts of the antioxidantproperties of taurine as a protecting material on tg-DNT induced toxicity in the liver and kidney in male rats. 100apparently healthy male rats in 4 groups were included; the first is Frank control group, mouth feeding via gavage withdistilled water; the other groups were administered as following, taurine alone, tg-DNT alone (toxic group), taurine + tgDNT (protective group) in the second, third and fourth groups, respectively. In these groups, blood biochemistry and taurineconcentrations in serum were measured for all animals. Furthermore, histopathological examination studies for liver andkidney were done for all groups. The results showed that, the protective group has marked improvement in most biochemicalparameters than the toxic group. Histological studies revealed a significant marked disturbance in the histopathologicalarchitectures of the kidney and liver in all toxic rats. However, marked improvements in histological architectures wereobserved in protective group. The results support the ameliorative effect of taurine as a protective agent against tg-DNTtoxicity in experimental rats.

ABSTRACT

Objective: To explore the effect of different processed products of rhubarb combined with atorvastatin (ATS) on the efficacy-enhancing and toxicity-reducing. Methods: L02 cells were used to investigate the hepatoprotective effects of raw rhubarb, wine-fried rhubarb and wine-steamed rhubarb in clinical application combined with ATS, and the extracts of the processed products with the best liver protection were screened out. The effective lipid-lowering dose range of the extract of the processed product was determined by HepG2 cell experiments; Finally, based on the model of hyperlipidemia in SD rats, the lipid-lowering and hepatoprotective effects of the optimal extract and ATS were generally evaluated by measuring the blood lipid and transaminase levels and related oxidative stress parameters, detecting the histopathological section of the liver. Results: The alcohol extract of wine-fried rhubarb in the three processed artillery products significantly improved the survival rate of L02 cells, and had the best protection effect on L02 cells induced by ATS. At the same time, the wine-fried rhubarb ethanol extract also effectively reduced the content of TC and TG in HepG2 hepatic steatosis cells. The liver-protection ratio and lipid-lowering ratio (mass ratio) of ATS to wine-fried rhubarb ethanol extract were 1:6.7-1:9.0 and 1:35.8-1:71.6. The wine-fried rhubarb ethanol extract and ATS reduced the blood lipid level and lipid content in the liver of hyperlipidemia rats; The serum levels of ALT and AST were significantly decreased, the SOD activity in the liver was significantly enhanced, and the MDA content was significantly reduced; And high-dose wine-fried rhubarb ethanol extract and ATS combination group significantly inhibited inflammatory cell infiltration and bile duct hyperplasia. Conclusion: The combination of wine-fried rhubarb ethanol extract and ATS can significantly improve the liver function damage caused by ATS, enhance the effect of lipid-lowering therapy on hyperlipidemic rats, and play a role in reducing toxicity and increasing efficiency.

ABSTRACT

The article analyzes domestic and foreign experience in planning the preparation of oral cavity for prosthetics. Recently, thanks to the development of computer technology, the possibility of virtual 3D-modeling of the prosthesis design and its further prototyping has appeared [1; 2; 5]. Digital treatment planning, based on three-dimensional imaging procedures, allows you to plan therapy with absolute precision and makes the outcome of treatment exactly predictable. Analysis of domestic experience shows that one of the most common in the practice of orthopedic dentistry is the program Blue Sky Plan. Patterns made in the program are easily exported to STL files and do not require additional processing. The program contains a wide range of implant systems. In foreign practice, guided surgery from DENTSPLY Implants is very popular, which offers a comprehensive solution for digital treatment planning and placement of implants based on SIMPLANT software templates.

ABSTRACT

Objective@#To investigate the association between the 5T site polymorphism of the cystic fibrosis transmembrane conductance regulator (CFTR) gene and the risk of congenital bilateral absence of the vas deferens (CBAVD).@*METHODS@#This case-control study included 40 male patients with isolated CBAVD in the experimental group and 104 healthy men as controls. We used the Sanger sequencing method to encode the CFTR gene intron 9 (TG) m-n(T) and type the haplotypes, followed by a review and meta-analysis of the data obtained from the experiment and relevant literature from the PubMed, Web of science, Medline, CNKI and an exploration of the correlation between 5T mutation and the risk of CBAVD.@*RESULTS@#Sanger sequencing revealed 6 genotypes in the CBAVD patients, including TG11-5T, TG12-5T, TG13-5T, TG11-7T, TG12-7T and TG11-9T, and 7 in the healthy controls, which were TG11-5T, TG12-5T, TG10-7T, TG11-7T, TG12-7T, TG13-7T and TG11-9T. Compared with the controls, the CBAVD patients showed obviously increased rates of the TG12-5T haplotype (4.81% [10/208] vs 16.25% [13/80]) and the TG13-5T haplotype (0% vs 7.5% [6/80]), but no significant difference in the TG11-5T haplotype (1.92% [4/208] vs 2.50% [2/80]). There was a statistically significant difference between the experimental and control groups in the TG12_13-5T haplotype (OR = 7.40, 95% CI: 4.83-11.34, P < 0.01). The TG12_13-5T haplotype was found to be highly correlated with CBAVD.@*CONCLUSIONS@#The haplotype of TG12_13-5T increases the risk of CBAVD in men, which has provided a theoretical basis for male reproduction.

ABSTRACT

@#Introduction: Thyroid disorders are among common medical conditions encountered in clinical practice. However, the disease burden among Malaysian population has not been established. This study was aimed to determine the prevalence of goitre and its associated factors among adult population in Peninsular Malaysia. Materials and Methods: A cross-sectional study was performed in 5 pre-assigned regions in Peninsular Malaysia. Participants’ sociodemographic profile and medical history were recorded. Physical examinations were done looking for abnormalities of the thyroid gland and signs of thyroid dysfunctions. The diagnosis of goitre was made by palpation and corroborated by the repeat examination by another investigator. The World Health Organization (WHO) goitre grading system was used. Fifteen millilitre of blood were withdrawn and analysed at a central laboratory. Results: Among 2190 respondents, the prevalence of goitre was 9.3%. Goitre was associated with positive anti-thyroperoxidase and anti-thyroglobulin antibodies. Females were at higher risk of goitre (adjusted OR = 2.4; 95% CI 1.6–3.5; p<0.001). The other risk factors of goitre were Indian ethnicity (adjusted OR=2.0; 95%CI 1.2–3.3; p=0.006), positive anti-thyroperoxidase (adjusted OR=1.8; 95% CI 1.1-3.1; p=0.024) and living in rural area (adjusted OR = 1.7; 95% CI 1.2–2.5; p=0.002). These areas had been shown to suffer from some degree of iodine insufficiency in the past. Conclusions: The prevalence of goitre in Peninsular Malaysia was comparable with other studies. The main predictors of thyroid disorders are female gender and thyroid antibodies. Living in rural areas was associated with goitre.
